â–  PATHWAY FOCUS: The Posterior Funiculus (Dorsal Columns - Medial Lemniscus pathway) is the primary pathway for high-fidelity sensory modalities, including fine discriminative touch, vibration sensation, and conscious proprioception. â–  SOMATOTOPIC ORGANS & COURSE: 1. Fasciculus Gracilis: Located medially. It carries sensory fibers from the lower limbs and lower trunk (below dermatome level T6). 2. Fasciculus Cuneatus: Located laterally. It carries sensory fibers from the upper limbs and upper trunk (above level T6). 3. Primary Ascending Course: Fibers enter the spinal cord and ascend ipsilaterally through the dorsal columns without crossing. 4. Medullary Synapse & Crossing: They synapse on second-order neurons in the Nucleus Gracilis and Nucleus Cuneatus of the caudal medulla. Axons of these second-order neurons cross to the opposite side as Internal Arcuate Fibers and assemble as the Medial Lemniscus, which projects to the thalamus (VPL). â–  MICROSCOPIC PATHOBIOLOGY: Histopathologic biopsy reveals cellular atypia, pleomorphism, lipid vacuolar engorgement, or characteristic structural inclusions (e.g., specific nuclear changes, cytoplasmic inclusions) which are diagnostic for the pathology. â–  GENOMIC VARIANT CHARACTERISTICS: Molecular profiling indicates that specific genetic subtypes exhibit varying levels of enzyme activity and drug-clearance efficiency.
