- Head/Neck: Sum of Erythema + Induration + Excoriation + Lichenification (0 to 12 scale)
- Head/Neck Area (0: 0%, 1: 1-9%, 2: 10-29%, 3: 30-49%, 4: 50-69%, 5: 70-89%, 6: 90-100%)
- Trunk: Severity Sum (0 to 12 scale)
- Trunk Area Grade (0-6 scale)
- Limbs: Severity Sum (0 to 12 scale)
- Limbs Area Grade (0-6 scale)
