- 1. Patient Age (years)
- 2. Clinical diagnosis of COPD or Chronic Bronchitis? (+2 Pts)
- 3. Family history of lung cancer? (+1 Pt)
- 4. Active or former smoker status
- 5. Cumulative pack-years (Packs per day × Smoking years)

Provides objective justification and shared decision metrics to authorize low-dose chest CT (LDCT) scans.
